Arizona Medical Marijuana Certification Centers

There are three easy steps to obtaining your Medical Marijuana Certification!
1. Your Medical Records State law requires us to obtain and review medical records dated within the last 12 months. Patients are encouraged to fax in or email their records or sign a medical release to be faxed to your doctor so we may request them for you.
2. Physicians Approval Once our staff receives your medical records we get them to our Physicians for review and approval.
3. Your Clinic Appointment Please bring your valid Arizona ID/DL or US Passport, EBT card (if on the S.N.A.P.program), a list of any medications you are currently taking as well as your payment in the form of cash, Visa/Mastercard, or Money Order made payable to AZMMCC.
