In the State of Colorado you must have at least one qualifying medical condition in order for a Physician to recommend Medical Cannabis for you. If you have one or more of the following debilitating conditions please contact us to schedule an appointment.
Medical Records
We ask if you have any medical records about your condition that you bring them with you to your appointment. Records may consist of your diagnosis, treatments, medication lists, X-rays, or any other testing you may have completed.
Don't worry, we will make copies of your records and make sure you get your originals back.
Our staff can also help you request your records from your doctor at no additional charge.
Don't have any records? Our Physicians are happy to consult with you about your condition but you may have a better chance of qualifying with medical record. Please call our office for more details, or send us an email in the box to the right.

Any patient with PTSD must have medical records at the time of your exam. We can only see adults with PTSD, and regardless of your age, medical records are required.
