Jerabek Wellness is a physician’s office that was originally started in 2017 by Dr. Jerabek and his passion to inform, educate, and treat individuals in the challenging specialties of Age Management, Functional Medicine, and Preventative Medicine. Through these passions, Dr. Jerabek brings unparalleled experience in issuing medical cannabis cards to patients through 21 years of practice.
