To get a medical marijuana card in the state of Arizona you must first obtain a Physician's Certification. This is a necessary document for application to the state of Arizona for the legal use of medical marijuana.
There are three easy steps to obtaining your Medical Marijuana Certification!
1. Your Medical Records
State law requires us to obtain and review medical records dated within the last 12 months. Patients are encouraged to fax in or email their records or sign a medical release to be faxed to your doctor so we may request them for you.
2. Physicians Approval
Once our staff receives your medical records we get them to our Physicians for review and approval.
3. Your Clinic Appointment
Please bring your valid Arizona ID/DL or US Passport, EBT card (if on the S.N.A.P.program), a list of any medications you are currently taking as well as your payment in the form of cash, Visa/Mastercard, or Money Order made payable to AZMMCC.

The primary mission of the Arizona Medical Marijuana Certification Center (AZMMCC) is to provide the certification necessary for qualified people to obtain Medical Marijuana under the State of Arizona Medical Marijuana Laws.

Patients who qualify for their Medical Marijuana Card with the Arizona Medical Marijuana Act after doctor review will receive a signed State Certification Form, Patient Attestation Form, Photo Taken and their Az Drivers Licence copied in the correct format to upload the application to the State of Arizona. Your Arizona Medical Marijuana Card will be mailed to you within 10 of submitting your application.
