Approved Qualifying Debilitating Medical Conditions: Cancer or the treatment of this condition Glaucoma or the treatment of this condition; Positive status for Human Immunodeficiency Virus (HIV) or the treatment of this condition Acquired immune deficiency syndrome (AIDS) or the treatment of this condition Hepatitis C or the treatment of this condition; A chronic or debilitating disease or medical condition or its treatment that produces one or more of the following: Cachexia or wasting syndrome; Severe, debilitating, chronic pain; Severe nausea; Seizures, including but not limited to those characteristic of epilepsy Severe and persistent muscle spasms, including but not limited to those characteristic of multiple sclerosis or Crohn’s disease; Agitation related to Alzheimer’s Disease.
The cost for an initial evaluation and/or initial Rhode Island renewal is $225. All follow-up Rhode Island renewals are $150. This will include a review of your medical records, complete non-invasive physical exam, maintenance of records for a minimum of five years, and a re-check appointment within one (1) year of the date on which your application was signed by the physician. The same cost applies to patients seeking a license renewal if it was signed by another practitioner. The fee for a renewal on our patients has not yet been determined due to changing legislation and requirements; however, it will be drastically lower than your initial fee.
Based on the Minimum Standards for Medical Marijuana the authorizing practitioner must, “be committed to the continual assessment of the patient and the patient’s response to the use of marijuana. This must be demonstrated through follow-up appointments, semi-annually at a minimum, before a card is renewed.”
License RenewalOur patients should submit another request appointment form on the website after their 10th month of being licensed. You will be scheduled for an appointment at no additional cost. If you fail to be reevaluated at the semi-annual schedule, you will need to reapply as a new patient. This is again based on standards set forth by the DOH. Therefore, if you miss any of the appointments below we are required to start the process over.
